Total Care Options provides ‘step down’ care to bridge the gap between hospital and home for clinically discharged patients following hospital treatment. We provide a focused level of care to aid recovery and rehabilitation, with wound management and condition monitoring, integrated with professional therapists, to stabilise and assess for discharge home. 

Our service reduces stays in hospital and prepares people for their return to home where we offer a re-enablement service, where necessary, to help relearn how to do daily activities, like cooking meals and washing.
